STUDENT IDENTIFICATION

 

 

A state identification number shall be assigned each student upon enrollment, in accordance with Louisiana Board of Elementary and Secondary Education requirements.

 

EMERGENCY INFORMATION

 

At the beginning of each school year, students shall be required to complete emergency information forms which shall contain such information as:

 

  1. Parent/guardian’s work phone numbers

  2. Home phone numbers

 

  1. Phone numbers where parents/guardians can be reached in case of emergencies

  2. The name, address, and phone number of the student’s physician

  3. The names of authorized persons to check-out and/or pick up students in the absence of the parent/guardian. 

  4. Other information as may be requested on the form
